Personal Information We Collect About You
Personal Information includes information we can use to specifically identify you such as:

technical device information (e.g. the Internet Protocol address for your device or device identifier);
Information you provide directly to Insulet via our Websites, via surveys or other communication platforms e.g., social media. You should ensure that you only provide necessary information relating to your specific request.
We collect Personal Information from you from various sources:
When you voluntarily choose to provide such information such as when you enter your Personal Information on our Websites or when you download and register or otherwise access an Insulet mobile application ("Application"). If you contact us, we may also keep a record of that contact.
Non-Identifiable Information/Aggregated Information:
When you interact with Insulet Corporation through our Websites, we may receive and store certain personally non-identifiable information. We may store such information ourselves, or it may be included in Information bases owned and maintained by affiliates, agents or service providers of Insulet Corporation.
